At your first appointment, we will collect your health history and any other dental records you may have. After your examination and X-rays, we will discuss the treatment options available, answer any questions you may have and make a follow-up appointment. Our administrative staff is ready to help you with questions about scheduling, financial policies and insurance to keep that part of the process as simple as possible.
WHAT YOU NEED.
Please provide the following information at the time of your consultation:
If you have x-rays taken by a previous dentist, please bring those with you.
Please bring your insurance card and any insurance information you may have.
Please be ready to provide your medical history and a list of medications you are currently taking.
As a courtesy to our patients, our practice accepts most dental insurance programs, including non-managed care, indemnity (traditional) and PPO out-of-network. We are not part of any managed care network. We prepare and file claims on your behalf. Dr. Melcher and Dr. Rodriguez are Delta Dental Premier Providers.
When you call to request an appointment or submit a form online, we do our very best to schedule you as promptly as possible. We offer a variety of dental treatments and procedures, therefore unforeseen circumstances may lengthen the time allocated for a procedure.
For your convenience, we accept most insurance plans, cash, personal checks, money orders and major credit cards. We offer convenient interest-free dental financing options that can help you get the treatment you want and need with affordable monthly payments through Care Credit.
